FFC Terms & Conditions

Paid in Full Registration Option Price is: $3800 for non-members, $2800 for members and $500 for employees.

*Fee includes all training seminars , testing (three), manual, (3) 50-minute private sessions, (4) group apparatus sessions and ability to use the facilities at FFC to gain apprentice hours.
*Apprentice must complete 450 hours of class time, self practice, observation and practice teaching.
Times: Friday sessions 2pm-6pm, Saturday sessions 12-5pm

CANCELLATION POLICY: I understand that if I cancel the program prior to the 3rd class of the program and have made one payment for the program, I will be refunded 50% of the program fee and retain my CPF Training Manual. If I paid for half of the program, I will not receive a refund but will retain my CPF Training Manual. After the completion of the 3rd class, program fee is non-refundable.

WAIVER: I have enrolled in a program(s) of educational knowledge and/or physical activity including, but not limited to, cardiovascular, strength and flexibility training. I hereby affirm that I am in good physical condition and do not suffer from any disability which would prevent or limit my participation in said programs. In consideration of my participation in this program, I fully understand that I may suffer injury and I, for myself, my heirs and assigns, hereby release Fitness Formula (their employees and owners) from any claims, demands and causes of action arising from such injury.

FFC Terms & Conditions

You have chosen the Payment Plan option for program enrollment. For non-members: $1500 deposit is due at registration, $1500 due 2nd month of program, $800 due 3rd month of program. Credit card is required to be placed on file for payment plans. We do not accept cash payment for payment plans. Member and Employee total fees may vary from above in price but follow the same billing cadence. Members will be charged through the same payment method as monthly dues billing.

FFC Terms & Conditions

I am registering for the FFC Pilates Teacher Training. I have paid $100 at registration and I am requesting the remaining cost of $400 be deducted from 8 of my paychecks starting the first month of the program. Each deduction will be $50.

I understand that should my check not be adequate enough to cover the $50, the remaining balance will roll to the next payroll period until I have covered the full expense of the course. I also understand that should my employment status change for any reason, I agree to pay any remaining balance through an alternative form.

Employee Agreement

FFC is covering $2,000 of the cost of this program for employees off of the regular price of $2,5000. You must be employed at least 6 months after the end date of the program or you will be responsible to pay the additional $2,000.
